Not sure where to ask this - DVD case & printed DVD

I have a wedding slideshow done and need a custom printed DVD and case. I'd rather not use an at home kit as they suck! Any ideas where I can get one made? I usually order through MPIX but it doesn't seem that they offer this! GRRR!!!
