In elements 4 you were able to see front, middle and back of card. I'm not sure how to see the separate sides of a create a card in elements 5... maybe I'm missign something?

I didn't know PSE4 had a "create a card" part to it.
When I make greeting cards, I usually made single fold. I make one canvas for the front and back and one canvas for the inside of the card.
I usually start with the finished size of my card - say 5x7. I make a "front" on a 5x7 blank canvas. When I'm done with my creation, I expand the canvas to 10x7 - keeping the "creation" in the bottom half or right half of the canvas. I print that 10x7.
Or you can start with the 10x7 and draw a line across the center - to visualize the front and back - deleting that "line layer" when it's time to print.
I then create the inside - reinsert the original print and print on the back.
I then fold it over.

For a quarter fold, I draw lines to divide my page in fours - and create in each section - using a "template" I've made to make sure I put the front back and inside in the proper quarters of the page.
To make a quarter fold template, take a sheet of paper - fold it in quarters - like a greeting card - then label your sections "front, back, verse" - use that template as a visual for creating your card with PSE.

I do my 4-fold cards on A4 in Elements 4 having made my notes as to what goes where, and what is upside down:
Greeting inside front
(upside down) (Upside down)
Back Front
I've been printing out my Christmas cards!
