It is very straightforward-when you open the Organizer in V5, it will automatically convert your catalog. It will automatically rename your old one to CatalogName-1.psa (CatalogName is My Catalog for default catalog; if you have multiple catalogs, it will convert each one as you open them).

The only recommendation I've seen is to open your catalog under V4 and do a File Reconnect All Missing (either reconnect or delete from catalog) and then a File>Catalog>Recover. This gives you a nice clean catalog to convert.
